Where the growth came from

Research & Development Solutions (R&D Solutions) led. Its revenue rose 8.7% to $2.64 billion, or 9.1% at constant currency. Excluding reimbursed expenses, it grew 6.8% as reported. Commercial Solutions revenue rose 6.9% to $1.79 billion, or 7.9% at constant currency.

R&D Solutions net new bookings were $3.2 billion, up 15% from a year earlier, a book-to-bill ratio of 1.2x. Over the last twelve months bookings were $11.7 billion, up 14%. CEO Ari Bousbib called it the strongest clinical quarterly bookings since 2022.

R&D Solutions contracted backlog was $32.6 billion on Sept. 30. The company expects about $9.4 billion to convert to revenue in the next twelve months, growth of 10% year over year. A review of inactive trials adjusted backlog by approximately $2.1 billion. The release says this does not affect historical results, 2026 guidance, book-to-bill or the twelve-month conversion estimate.

Why it matters

OptimistFi has argued that IQVIA compounds if drugmakers and healthcare customers keep outsourcing clinical development and commercial intelligence to a scaled, data-rich partner. This quarter supports that half of the view. Bookings are growing faster than revenue, and the twelve-month backlog conversion estimate is growing at 10%.

Revenue for the first nine months of 2026 was $12.95 billion, $1.00 billion above the $11.95 billion of the same period in 2025. That is growth of 8.4% reported and 7.7% at constant currency. The quarter fits a steady run, not a one-quarter spike.

Not all of it is organic. Revenue guidance includes 250 basis points from acquisitions, including deals the company expects to complete by year-end, plus about 20 basis points of foreign-exchange tailwind. The backlog review is a reminder that backlog figures are estimates.

Earnings per share grew faster than profit. Adjusted Net Income rose to $557 million from $515 million, while diluted shares fell to 167.0 million from 171.7 million. Adjusted EBITDA also adds back stock-based compensation of $83 million, up from $55 million, and restructuring and related expenses of $41 million, up from $30 million.

Cash flow and debt

Free cash flow was strong against earnings but weaker than last year. It was $667 million, 120% of Adjusted Net Income, down from $772 million a year earlier. Operating cash flow was $825 million versus $908 million. For nine months, free cash flow was $1.52 billion versus $1.49 billion.

Debt was $16.07 billion against cash of $2.25 billion. That leaves net debt of $13.82 billion, or 3.45x trailing-twelve-month Adjusted EBITDA. IQVIA repurchased $950 million of common stock in the first nine months of 2026.

What's next

IQVIA will hold an Investor Day on Tuesday, Dec. 1, 2026. Presentations from senior executives run from 9:00 a.m. ET to about 12:30 p.m. ET. Management says it will update the business. Registration is required.

The test is whether organic growth holds as acquisition revenue enters the numbers, and whether the twelve-month backlog conversion estimate stays on its 10% growth path. Slower bookings or a lower conversion estimate would weaken the view.
